This happened in a restaurant I used to work in. It was Autumn, at that time of year when the world is awash with Daddy Long Legs. :P Unfortunately, one such creature found its way into a lady's plate of linguine.

The waiter apologised most assiduously. The manager was called and, being a nice bloke offered the couple a fresh plate and the whole meal on the house. His co-manager (his rather strident wife) was not impressed with him giving the whole meal away and they proceeded to have an almighty row across tbe bar where I was stationed.

I still think he did the right thing (the free meal, not the argument with his wife).
